Visa & Passport Information
A passport and visa may be required for your upcoming trip. Travel document requirements and rules can be tricky. What’s more, they can change daily. The information below will help you figure out which travel documents you’ll need to enter the countries you’re visiting, and to get back into the country where you started.
RUSSIAN VISA REQUIREMENTS
Travel to Russia requires an entry visa. The Consulate General issues different categories of visas to Russia based upon the purpose of the trip and the duration of stay in Russia. The following documents should be submitted in order to obtain a visa according to its category:
- Completed visa applications (two for a multiple re-entry visa), signed by the applicant only. All information must be typed or printed in block letters in English. Incomplete visa application forms shall not be processed.Click here to download an application.A new visa application form was put into effect June 15, 2009. Complete it and print it out according to the requirements enclosed. Each application form must be signed by the visa applicant personally.
- Valid national passport (original only and it should have at least two clear visa pages). It must be valid no less than six months after the visa expiration date. Holders of travel documents such us Permit to Re-Enter the United States of America, Travel Document, etc. must submit valid Permanent Resident Card (an original and a copy).
- Two pictures of an applicant.
- Money Order or Certified Bank Check made out to the Russian Consulate for visa processing. Please note that we do not accept cash, credit or debit cards, personal or company checks.
- Visa fee: Since visa processing fee is subject to be changed, please call 718-645-8578 for updated information.To obtain a visa by mail, the request must include a postage paid return envelope. AZERBAIJAN VISA REQUIREMENTS
To obtain a visa for travel to the Republic of Azerbaijan, the following documents must be submitted to the Consular Section of the Embassy:
- One completed visa application form. Application form is available through the Embassy website www.azembassy.us or may be faxed upon request. (Please, answer all questions by typing or clearly printing in ink on both left and right sides of the application.)Click here to download an application.
- Original passport or travel document (photocopies will not be accepted).
- Two passport-size photos.

All foreigners visiting Azerbaijan for a period of more than 30 days must register their passports with the local passport-registration authorities.
UKRAINE VISA REQUIREMENTS
Citizens of the United States, European Union, Swiss Confederation, Liechtenstein, Canada, and Japan DO NOT need visas for entering the Ukraine anymore. The rule is valid for short term visits, up to 90 days.For obtaining a tourist visa to the Ukraine for citizens of other countries, the following will be required:

Visa Tips:
- It is advisable to apply for a visa at least one month prior to your prospective departure date.
- If needed, a Consular officer may request additional documents that would better specify the purpose of the alien’s trip or invite a certain person for an additional interview.
- Visa processing starts only after the consulate has registered your application and collected all necessary documents. A drop-off day is not counted as a business day.
- Any application received on weekends or official U.S. holidays will be registered by the Consulate only on the next business day.
- ALL APPLICATION PAPERS MUST BE CORRECTLY AND COMPLETELY FILLED OUT (as well as a proper payment attached). FAILURE TO COMPLY OR PROVIDING INCORRECT OR MISLEADING INFORMATION WILL RESULT IN THE RETURN OF THE APPLICATION UNPROCESSED.
